I have been using O'Neill rash guards for over 15 years and this rash guard is awesome!
I have three of these POLYESTER rash guards (and just ordered a fourth) in different colors. I find the SIZING to be ACCURATE - rash guards are intended to fit skin tight (hence the name "SKINS").
The polyester/elastane material this rash guard is made of does not stretch as much as the nylon/spandex material some rash guards are made of. BUT, this polyester rash guard / skin has lasted longer and does very well with daily chlorine exposure from swimming pools. So, I go with slightly less stretch ; overall comfort in exchange for durability.
If you're concerned about the sizing, the picture shows exactly how the rash guard is intended to fit - like a second skin. Generally, rash guards are sized to fit tight so the material will not bunch-up or slip/slide around during water sports or when worn under a wetsuit. If your preference is for a looser fit, just size up or get a "rash tee" instead. Also, the sizing is definitely not designed for plus-size ladies or big girls with big girls.
Some of the other reviewers don't know how a rash guard is meant to fit and have stated that the sizing is off from their own ignorance and/or preference for a looser fit. One reviewer gives the impression that they thought they would get a plus-size fit (known in certain stores as "Women's Sizes - in other words "Plus Sizes"). Another reviewer who left a bad review also left a bad review for another product (a wetsuit skin from a different manufacturer) with the exact same title - word-for-word - stating that the sizing for that product was off also.
What's going on?
If your preference is for a looser fit (from a product that is meant to fit tight), just do some research on how rash guards are meant to fit as well as the material it's made out of and just size-up instead of leaving yet another bad review out of ignorance.
O'Neill puts a lot of effort into creating quality rash guards and wetsuits - give them some respect and common courtesy.

UPDATE After about a month of almost daily use in residential swimming pools the stitching has faded slightly and the lettering on the front is cracking slightly. Color of the rash guard / skin (the polyester material) has stayed the same.

Atucally purchased this for a little extra warmth while snorkling because I didnt want to get a full wetsuit. Was perfect with 68 degree water kept me warm and comfortable not having to wear too much.
